Senior Transportation Survey

One of the functions of the Jewish Community of Louisville is to convene community leaders to look at the long-term needs of the Jewish community and to develop plans to address them. To accomplish this task, the JCL Community Planning Forum, comprised of lay and professional leaders of Louisville’s major Jewish agencies and congregations, meets periodically.

 

Recently, the members of the Community Planning Forum identified the transportation needs of seniors as an important community issue. To clarify the issue, they agreed to conduct a Senior Transportation Needs Survey.

Seniors and current and potential caregivers are encouraged to fill out the survey on this page and to send it to Jewish Family & Career Services, 2821 Klempner Way, Louisville, KY 40205, by August 31. This form can also be faxed to 452-6718. Additional survey forms will be available from Louisville’s Jewish agencies, organizations and congregations.
